First appointment

Request at least two weeks in advance to have a copy of your medical records sent to your new adult gastroenterologist.  Bring your own copy, your insurance card and your Journey Summary which is part of the MyIBD website.


Below is a short list of things to gather and bring when you have an appointment with your gastroenterologist.



  • Your medical records
  • Your health/medical insurance card
  • A list of your current medications
  • Your pharmacy’s address and phone number
  • Your other doctors’ names and contact information.


Have Knowledge:

  • Know what address you are going to and do a quick Google map search before leaving to understand how long it will take you to get there. Keep their phone number in case you are late or need help finding the right place.
  • Ask for the doctor’s room number and floor
